Bilingual candidates are encouraged to apply. This is a strong fit for a clinician who values autonomy, flexibility, one-on-one patient care, and meaningful community-based work.You will evaluate patients in the home, build and direct treatment plans, and help improve mobility, gait, strength, balance, ROM, and functional independence. You will also educate patients and caregivers, coordinate with physicians and the care team, complete documentation, and support discharge planning as needed.ResponsibilitiesPerform in-home physical therapy evaluationsDevelop and direct individualized treatment plansAssess gait, strength, mobility, ROM, and rehab potentialProvide patient education and caregiver instructionCoordinate care with physicians and interdisciplinary team membersComplete documentation, progress summaries, and discharge planningEvaluate home environment and safety needsSupervise PTA support when applicableRequirementsBilingual: Russian, Mandarin, or Cantonese speaking Licensed Physical Therapist in New YorkDegree from an accredited Physical Therapy programMinimum 1 year of PT experienceHome health or community-based experience preferredStrong communication and documentation skillsCurrent CPR certificationInterested in hearing more?
