Learn more about SWACH at www.southwestach.org HealthConnect Hub includes a regional cohort of community-based workforce, a broad network of community and clinical stakeholders, and a technology platform that supports whole-person care for community members through connection to the community-based workforce. The Systems Specialist is accountable for coordinating, integrating, and connecting Community Health Records (CHRs) and PeaceHealth Southwest referrals across multiple referral partners and community-based care coordination programs with the assurance of accurate decision-making. This position is also responsible for managing and auditing PeaceHealth Southwest’s Community Health Hub client caseloads for quality and program integrity. Essential Duties and Responsibilities: System Management and Referrals – 80% · Support and help design new processes and changes to screening and referral systems · Provide monitoring and QA for referral engagement and outcomes capture · Run monthly Quality Assurance (QA) reporting · Manages PeaceHealth Southwest’s Community Health Hub client caseloads: o Manages enrollment, discharge, and client transfer o Data audits and cleanup processes o Support and coordinate user management, records management, consent management for HealthConnect Operations. o Provide communication support for agencies, partners, and programs working together · Assist HealthConnect team members with additional referral projects/ backlog/coverage · Support overall HealthConnect operations for efficiency and capacity support Training and Technical Assistance – 20% · Provide orientation and training to users in the system · Provides regular technical assistance to the PeaceHealth Southwest’s Community Health Hub network of users. Work with CHR System Specialist to implement changes in the CHR · Customer Service for technical support within the PeaceHealth Southwest’s Community Health Hub · Represents SWACH and treat all program partners with integrity and respect. · Other duties as assigned. Administrative and Inclusivity · Advance equity in projects and across the organization · Prioritize work and manage time on an ongoing basis · Represent SWACH and all program partners with integrity and respect · Foster an inclusive culture of trust, respect, and mutual accountability · Role model of SWACH’s vision, mission, and values to all staff · Other duties as required To be successful in this role, you should have the following qualifications: · Ability to manage and analyze data systems · Ability to develop and conduct training and group presentations · Ability to support work as it evolves based on community and organizational needs · Ability to work successfully independently and as a part of a team · Demonstrated problem-solving skills · Ability to establish and maintain effective working relationships with individuals from various groups and backgrounds, including team members and community members · Understand and fully support the notion that people in marginalized communities are the experts about their own lives and have the best solutions to their own problems. · Experience with MS Office applications (Word, Excel, Outlook, PowerPoint, MS Project) · Proven ability to maintain high ethical standards and confidentiality Qualifications-Education and Experience Any combination of education and experience that demonstrates the ability to perform the duties of the position is qualifying. This would typically include: · Bachelor’s degree from accredited college/university, or equivalent combination of education and experience with a focus in health / community health/ data · Experience with data systems including data analysis, understanding, and reporting preferred · Three to five years of experience working in social service or clinical settings. · Experience understanding and addressing health equity and social determinants of health in diverse, underserved communities preferred. · Ability to pass background check and 10-panel drug screening Working Conditions & Physical Requirements Work is performed both virtually and in-person in an office setting located at PeaceHealth Southwest and at SWACH office as needed for team meetings, 1:1s, All-Staff meetings and trainings. Potential for travel among the three-county service area is required, including occasional evening and weekend meetings. Ability to spend long periods working on a computer and using office equipment. Using the telephone, virtual platforms, and/or face-to-face meetings to connect and collaborate with others. Drafting, evaluating, editing, and reading documents.
