JOB REQUIREMENTS: Community Horizons is assisting a member to find acaregiver to provide support that will help ensure quality of life whileremaining in her home. She is a kind person who enjoys knitting,visiting, and attending church. This is a self-directed care roll. Youcan help have a huge impact on her life by providing short visits 4times/week.
