LPN/RN MDS Coordinator

Garden Spring Center is seeking a detail-oriented and dedicated Licensed Nurse Assessment Coordinator (LNAC) / LPN MDS Coordinator to join our team. This role is responsible for coordinating and completing the Minimum Data Set (MDS) assessments in compliance with federal and state regulations, ensuring accurate reimbursement and high-quality resident care.Key Responsibilities:Coordinate and complete MDS assessments (OBRA, PPS, and managed care) in a timely and accurate mannerEnsure compliance with all CMS, state, and facility guidelinesCollaborate with interdisciplinary team members to gather and verify resident dataMonitor and track assessment schedules to meet required deadlinesParticipate in care planning and ensure MDS reflects individualized resident needsMaintain thorough documentation to support coding and reimbursementAssist with audits and implement corrective actions as neededStay current on regulatory changes and MDS best practicesQualifications:Active LPN license in good standingCertification as a Resident Assessment Coordinator (RAC-CT) preferred (or willingness to obtain)Previous MDS experience in a skilled nursing facility strongly preferredStrong knowledge of CMS guidelines and reimbursement systemsExcellent organizational, communication, and analytical skillsAbility to work independently and as part of a teamBenefits:Competitive salaryHealth, dental, and vision insurancePaid time offOpportunities for professional growth and developmentWhy Join Garden Spring Center?At Garden Spring Center, we are committed to providing exceptional care to our residents while fostering a supportive and collaborative work environment for our staff.
