QMHS/Case Manager
Position Title: QMHS Support StaffReports To: Clinical Manager/Clinical DirectorPurpose: Under the direction of a clinical manager, the worker provides support to consumers and their families or caregivers with mental health related issues. The work will utilize intervention and activities that are strength-based and focused on promoting recovery, symptom reduction, while increasing coping skills and help to achieve the highest level of functioning in the community.POSITION RESPONSIBLITIES:Ensure compliance with all policies and procedures, as well as local, state, and federal regulations. The worker is responsible for providing medical necessary services to the consumer(s) in accordance with the treatment plan and diagnostic assessment that is designed to meet the mental health, educational, vocational, residential, financial, social and other needs of the consumer.Follow Mental Health Services guidelines and ensure that treatment plan authorizations and reauthorizations, and updated diagnostic assessments and all other assessments are current and do not lapse or expireServe as an intricate member of the service team to ensure continuity of care and supports the services 24 hours a day, seven days per week as necessaryProvide structured face-to-face scheduled sessions to consumer at least one per week or as needed based on the treatment plansProvide services in the home, school, office and community settingEnsure that the majority of mental health services are primarily rendered in the community and ensure that limited services remain clinic basedDocument encounters within 24-48 hours of service or phone callReport to supervision and in-house trainings as scheduledFacilitate or co-facilitate staff meetings and trainingsWork collaboratively with and provide ongoing updates to the clinical team including psychiatrist as neededIdentify, establish, and continually collaborate with community agencies, providers, and natural supports and advise team to make referrals as neededListen/respond to; interact with clients, co-workers, management, and other professionals in a non-judgmental mannerCommunicate objectively, thoroughly, accurately, concisely in a manner consistent with the cognitive level of the persons involvedDemonstrate an understanding and acceptance of the organizational mission and shared purpose to assist individuals by providing quality servicesParticipate in efforts to enhance service provision to consumersAct and interact to build cohesivenessAssist with building a positive image for the agency by demonstrating support and cooperation with other units and community agenciesDemonstrate professional work ethics as evidenced by overall dependability, conscientiousness, and timelinessEnsure all necessary licenses and/or certifications remain currentOther duties as assigned. MINIMUM QUALIFICATIONS: Master's Degree in Social Work, Psychology or other clinical based discipline preferredBachelor's Degree from an accredited institution with at least two years experience working in the field of mental health requiredMust be able to pass a satisfactory background check, drug test, and child abuse registryAbility to successfully complete service-specific training coursesProficient in clinical theories and their practical applicationComputer literate; experience with MS Word and ExcelPREFERRED QUALITIES: Successful applicant will be well organized, have excellent time management skills and be able to work well both independently and with a teamHave a valid drivers' license, reliable transportation and insurance
