Work Dates Needed: ASAP - 3 months Preferred Schedule: Monday through Friday 8a-5p 1 in 4 call rotation which is all phone based Worksite Setting: PACE outpatient center based with some in-home visits Scope of Work: Diagnosis and treat nursing home eligible senior population who desires to remain living independently, function as part of the interdisciplinary team to care plan for agreed upon participant goal outcomes 5 to 10 patients a day Licenses, Certifications, Requirements: Michigan licensure, ACLS/BLS, geriatric population experience required, PACE experience required EMR: CollabriOS PACE Care Online (PCO) Rates: $90/hour by Jobble
