Your recruiter will provide the most up-to-date details during the hiring process._**How you'll make an impact in this role**+ Lead patient care journeys by implementing individualized care plans that prioritize recovery and clinical excellence.+ Coordinate multidisciplinary teams as the central point of contact to ensure seamless, holistic patient support.+ Advocate for patient safety by monitoring health status and communicating critical changes for immediate, proactive intervention.+ Empower patients for life after discharge through collaborative transition planning that ensures long-term wellness.**What minimum requirements you'll need**Licensure / Certification / Registration:+ BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
