Your recruiter will provide the most up-to-date details during the hiring process.How you'll make an impact in this role* Lead patient care journeys by implementing individualized care plans that prioritize recovery and clinical excellence.* Coordinate multidisciplinary teams as the central point of contact to ensure seamless, holistic patient support.* Advocate for patient safety by monitoring health status and communicating critical changes for immediate, proactive intervention.* Empower patients for life after discharge through collaborative transition planning that ensures long-term wellness.What minimum requirements you'll needLicensure / Certification / Registration:* BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
