Care Coordination Program Coordinator
TriSource Staffing is seeking an energetic, dynamic, and passionate Lead Care Coordination Program Coordinator to join our team! In this pivotal role, you will work in partnership with and under the direction of the Care Coordination Program Director to support the effective delivery of Care Coordination Wraparound services across all program locations.
Serving as a lead team member and key support to the Program Director, you will provide guidance, mentoring, training, and operational assistance to Care Coordinators while helping ensure quality service delivery, regulatory compliance, and program effectiveness. (Note: This is not a supervisory position, but rather a subject matter resource, coach, and support role.)

Salary: $55,000 - $60,000 annually
Minimum Qualifications
Education: Bachelor's Degree preferred.
Experience: Experience working with children, youth, and families in behavioral health, human services, social services, or a related field preferred.
Must Have Driver's License and Personal Vehcile
Key Responsibilities
Program Leadership & Support
Support daily operations and implementation of Care Coordination Wraparound services.
Serve as a resource, mentor, and coach to Care Coordinators across multiple program locations.
Assist with onboarding, training, and ongoing professional development of staff.
Promote consistency in service delivery, documentation, and adherence to program standards and best practices.
Travel between program locations as needed to provide operational support, training, and program assistance.
Participate in team meetings, case reviews, and program planning activities.
Assist with quality improvement initiatives, program development efforts, community outreach, and the preparation of weekly unit sheets and reports.
Direct Service Responsibilities
Maintain a manageable caseload of no more than fifteen (15) Care Coordination clients.
Provide case management services in accordance with Service Provision Level I or Level II requirements.
Ensure services are coordinated appropriately without duplication.
Process initial authorizations and related documentation as needed.
Facilitate seamless coordination among service providers, families, schools, healthcare providers, and community partners.
Compliance & Quality Assurance
Maintain compliance with network, COMAR, CSA, BHA, CARF, and other regulatory standards.
Support preparation and ongoing maintenance of CARF accreditation activities.
Facilitate interagency program service delivery and participate in documentation audits and case reviews.
Identify opportunities for continuous quality improvement and assist with corrective actions.
