Brinton Manor Nursing & Rehabilitation Center of Glen Mills, PA is looking for an experienced RNAC/LNAC with a proven track-record of success to join and support our nursing team!The Company:We are a strong and supportive team of professionals and consider ourselves to be familyWe offer short term rehabilitative services and long-term nursing care with a 92-bed capacityWe provide exemplary customer service and top-quality care to each resident with our person-centered approachWe are committed to team-oriented outcomes and quality careWe show value and respect to our employees by offering opportunities for career advancement and promotion from withinThe Responsibilities & Duties:Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of RAP's and TriggersMaintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.Participate in facility surveys made by authorized government agenciesEvaluate each resident's condition and pertinent medical data to determine needsDevelop and implement procedure with the DON to inform all assessment team members of the arrival of newly admitted residentContact and assist in scheduling participation by outside members of the care plan team, including the resident's representative and/or other interested family membersDevelop preliminary and comprehensive assessment of the nursing needs of each resident, utilizing the forms requiredEnsure that the assessment team is aware of the importance of completeness and accuracy in their assessment function and ensure awareness of penaltiesThe Qualifications:Licensed RN in the state of PA2 years RNAC experience in a long-term care setting, requiredManaged care and bundled payment knowledgePDPM and MDS experience requiredPossess excellent oral and written communication skills with computer literacy, requiredExtensive knowledge of state and federal regulations and state survey process, requiredAbility to provide adequate training, oversight and correction actionThe benefits for qualified employees:Health & Dental InsuranceGenerous paid time off (PTO) as well as paid holidaysTuition reimbursementContinuous professional and clinical trainingAwards and recognition programsWe are proud to be an Equal Employment Opportunity employer.
