Our Social Work and Community Support Service (SWCS) delivers personalised, integrated support to individuals, families, and carers across multiple regions.This post is working 30 hours per week. This is a hybrid role, with at least 40% of your time based in one of our community settings.About The RoleWe are looking for a dedicated and skilled Social Worker to join our Social Work and Community Support team.In this impactful role, you will work with individuals and carers facing challenges such as ageing, ill health, disability, mental health needs, or social isolation. You will provide professional social work support, including assessment, care planning, advocacy, and safeguarding, enabling people to maintain independence and improve their quality of life.You will also play a key role in developing services, contributing to community initiatives, and representing Jewish Care within multi-disciplinary partnerships.This post is working 30 hours per week.This is a hybrid role, with at least 40% of your time based in one of our community settings.Key ResponsibilitiesManage a caseload of approximately 35 cases (FTE), delivering high-quality, person-centred social work interventionsUndertake holistic assessments, care planning, and risk managementWork in partnership with individuals, carers, and statutory and voluntary servicesProvide advice, information, and advocacy in line with current legislation and guidancePlay an active role in multi-disciplinary working and partnership developmentSupport the development of community and group-based interventionsPromote independence, choice, and dignity for individuals and familiesRepresent the service at meetings, forums, and community settingsMaintain accurate and timely case records using the organisation’s CRM systemContribute to the development and continuous improvement of servicesTriage and respond to referrals, assessing urgency and needApply professional judgement to make timely, effective decisionsMaintain an up-to-date CPD portfolio and engage in ongoing professional developmentAbout YouWe are looking for a compassionate, confident, and reflective practitioner who is committed to delivering excellent social work practice.EssentialQualified Social Worker with current registration with Social Work EnglandExperience working with adults in a social care settingStrong experience supporting people with mental health needs, disability, dementia, or complex needsSound knowledge of Safeguarding, the Care Act (2014), and the Mental Capacity Act (2005)Experience of community and/or group workExcellent communication, assessment, and IT skillsAbility to manage a complex caseload and make evidence-based decisionsStrong understanding of professional ethics, confidentiality, and safeguarding responsibilitiesAbility to work both independently and as part of a multi-disciplinary teamCommitment to learning about and engaging with the Jewish community and cultureDesirableExperience working within the voluntary sectorExperience in mental health servicesKnowledge of the Holocaust survivor community and related needsAwareness of Claims Conference processesUK driving licence and access to a vehicleJewish Care is the largest provider of health and social care services for the Jewish community in the UK.
