Job Description StaffMed Health Partners is recruiting a Post-Acute Care Physician for a leading national physician services organization in a SNF setting in Jackson, MI.Perks and Benefits: Compensation: Competitive base salary commensurate with experience, plus productivity and quality incentives Employed W-2 position with America's largest physician services organization Schedule: Full-Time (FT) - Full time (4-5 days/week) Productivity bonus and quality incentives on top of base Sign-on bonus and relocation allowance Comprehensive medical, dental, vision, and life insurance 4 weeks PTO plus paid holidays Paid malpractice with tail coverage Annual CME allowance plus paid CME days 401(k) with employer match Strong leadership pathway (Facility Medical Director, Regional Medical Director, CMO) Dedicated administrative, billing, credentialing, and scheduling support Clinician wellness program and responsive regional management On-Call: Yes Position Highlights: Provide attending physician services to a SNF resident panel (typically 75-125 patients) Conduct admission H&Ps within 48-72 hours, monthly recertifications, and significant-change-of-condition visits Manage chronic disease, post-acute recovery, wound care, dementia care, polypharmacy, and end-of-life care Lead and collaborate with NPs/PAs, nursing, therapy (PT/OT/SLP), social work, and consultant pharmacy Participate in interdisciplinary care planning, family conferences, and Medicare Part A clinical reviews Document in PointClickCare or facility EMR per CMS PDPM standards Qualifications: BC/BE in Internal Medicine, Family Medicine, or Geriatrics Comfort managing CHF, COPD, diabetes, dementia, palliative needs, and complex polypharmacy Interest or prior experience in long-term care, sub-acute rehab, or skilled nursing strongly preferred Active state license (or eligible) and DEA where applicable Board Certification: yes About Jackson, MI: Jackson is in south-central Michigan, between Ann Arbor and Lansing.
