North Carolina Licensed Case Manager

A company is looking for a Case Manager. Key Responsibilities Coordinate care, resources, and services for members to achieve optimal clinical and resource outcomes Develop and implement member-centric care plans, addressing barriers and utilizing community resources Monitor and evaluate member progress, adjusting plans as needed and documenting all aspects of care Required Qualifications Registered Nurse (RN) with 3 years of clinical and/or case management experience Valid and applicable clinical license (NC or compact multi-state licensure) Additional specialty certification (i.e., CCM, CDCES) may be required for some roles Ability to perform comprehensive assessments and collaborate with healthcare teams Experience in evaluating care provided across the continuum of care
