Addiction Counselor/Case Manager FT / PT

ESSENTIAL DUTIES AND RESPONSIBILITIES: Ensure that all clients are treated with dignity and respect. Affirming care for Lesbian, Gay, Bisexual, Transgender and Questioning clients. Provide group, individual, and family counseling. Complete assessments, admissions, and treatment plans (TPRs). Document all services provided. Maintain contact with referral sources (EAPs, Criminal Justice). Send Referral Progress Reports. Participate in weekly case conferences, supervision, and staff meetings. Ensure compliance with Seafield policies and confidentiality regulations. Complete discharge planning, UR, and referrals. Perform other duties as assigned. COMPETENCY REQUIREMENTS: Assessment & Referral Treatment Planning & Review Group, Individual, & Family Counseling Didactic Presentation Documentation Crisis Intervention Age-Specific Skills (Adolescents & Geriatric) Knowledge of Medication-Assisted Treatment EDUCATION & QUALIFICATIONS: CASAC or CASAC eligible, or B/S-B/A with working knowledge of substance abuse disorders.
