Case Manager

Position Overview: We are seeking a professional, organized, and compassionate Case Manager to join our residential treatment team in a home-based setting serving children and teens. This role is responsible for coordinating care, developing and implementing trauma-informed treatment plans, and ensuring each youth receives individualized, consistent, and high-quality support. Key Responsibilities:Develop, implement, and monitor individualized, trauma-informed treatment plans for youthCoordinate services with therapists, medical providers, schools, and other stakeholdersMaintain accurate and timely documentation, case notes, and reportsMonitor client progress and adjust plans as neededFacilitate team meetings and case reviewsAdvocate for the needs and well-being of each childEnsure compliance with program policies, state regulations, and best practicesProvide support and guidance to direct care staff regarding treatment goals and interventionsQualifications:Minimum of 3+ years of case management experience working with children and/or teensExperience developing and implementing trauma-informed treatment plans requiredStrong organizational, documentation, and time-management skillsExcellent communication and collaboration abilitiesAbility to maintain professionalism in a dynamic residential environmentMust be able to work in or commute to Rexburg and Rigby, IDBachelor’s degree in social work, psychology, human services, or a related field.Schedule & Work Environment:Residential treatment facility operating in a home settingMay require flexible scheduling based on client needsSome evenings, weekends, or on-call responsibilities may be required Pay: $24.00 - $30.00 per hourExperience:Case managment: 3 years (Preferred)
