Physical Therapist

Fishkill Center for Rehabilitation and Nursing - Fishkill Center for Rehabilitation and Nursing is seeking a Full time and Weekend per diem Physical Therapist (PT) to join our growing Rehab team. The PT provides skilled physical therapy services in our facility center consistent with professional standards and policies. The PT in our facility examines Residents upon admission, and then develops/administers therapy plans to manage their conditions, prevent further disabilities, and achieve long-term health benefits.Duties:Develops an overall therapy plan of care to include treatment and medical diagnosis of the warranted treatment.Plans duration and frequency of therapy to be provided.Utilizes standardized assessments upon the development of the plan of care to validate and monitor the need for skilled therapy services, and for the ongoing need for skilled therapy and the patient's progress from received.Completes all documentation required of observations, patient 's response to treatment, treatment programs, degree of progress and notable changes in patient's condition within required time in accordance with state and federal regulations and general standards of practiceCompletes all billing required on the date of the provided treatmentCommunicates and coordinates patient's care with other members of the patient' s health careSupervises physical therapy assistants in accordance with regulations and standards of care.Help Residents to improve their mobility and pain relief by using exercises, hands-on therapy, stretching, and physical therapy devices.Communicates with Residents progress and projected achievement of stages of their recovery plan.QUALIFICATIONS:Graduate of an accredited Physical Therapy ProgramCurrent New York State PT LicenseExperience in skilled nursing/ long term care settings preferredWe offer the Following:Competitive ratesWeekly pay401(k) matchingA comprehensive, outstanding benefit package including generous vacation, holiday pay, health, dental, vision insurance with employer contribution and reimbursement.Health, Dental and Vision at 24 hours/week.ABOUT US:We are a premier skilled nursing facility located in Dutchess County, New York dedicated to provide outstanding care to our residents in a warm, nurturing environment that allows each resident to maintain his or her individuality and dignity.
