The LMSW will conduct psychosocial assessments, provide supportive therapy, coordinate discharge planning, and facilitate referrals to appropriate community resources and post-acute services. Responsibilities:Conduct comprehensive psychosocial assessmentsProvide supportive counseling and therapy to patients and familiesDevelop and coordinate discharge plansFacilitate referrals to community agencies and support servicesCollaborate with interdisciplinary healthcare teamsMaintain accurate and timely documentationRequirements:Master's Degree in Social WorkValid LMSW license issued by NYSED (Primary Source Verification required)Minimum 1 year of LMSW experienceHospital experience requiredInpatient medicine, surgery (med/surg), or rehab experience preferredContract Details:Local and Travel Contract both availableTravel Pay: Up to $2,800/weekDay Shift: 9:00 AM – 5:00 PM
