To support case managers and healthcare professionals by performing a variety of tasks that range from administrative duties to patient interactions. To facilitate and coordinate a safe and appropriate discharge plan while meeting the patient's individual needs as well as state and federal regulations. To work collaboratively with multiple other disciplines throughout the organization and community. To manage patient records and coordinate followup care. To enhance the patient experience and optimizing the overall healthcare delivery process.Assist RNCC (RN-CM), SW-CM, and SWs in assessing, coordinating care, and developing and implementing discharge plans based on patient needs. Documents planning and interventions appropriately and timely. Communicates appropriate information between physicians, nursing units, administration and other disciplines to facilitate care transitions to ensure proper patient flow through the hospital system. Identifies any barriers and provides clarity to determine realistic goals for the discharge plan. Coordinate with patients and providers to arrange or facilitate follow up appointments (service based/specialty services), such as PCP for DME needs, and other post-discharge care needs. Maintain accurate and updated patient notes and referrals/packets in Careport or to contacts outside of Careport. Provide administrative support such as data entry, filing, document preparation, and patient signatures. Monitor patient progress and report updates to the RNCC (RN-CM), SW-CM, or SW. Assist licensed staff with facility discharges as needed by updating/modifying plans upon reevaluation and as delegated by licensed staff. Help patients complete necessary paperwork and applications for services. Communicate and coordinate post-discharge needs with community organizations and agencies for resource referrals. Facilitate follow-ups and check-ins with patients and other disciplines to ensure service and patient satisfaction. Performs other duties as assigned.QualificationsBachelor's degree in social work, psychology, sociology, or other healthcare related field.
