We are seeking a Social Care Navigator to help individuals access community resources, benefits, and services that improve health outcomes and support independence.Position Overview:At HDSW the Social Care Navigator (SCN) works directly with individuals to identify needs, and social support that promote stability, wellness, and independence. This role bridges the gap between clients and the complex social service systemsKey Responsibilities:Conduct person-centered, strengths-based assessments to identify needs related to housing, food access, nutrition and transportationFacilitate referrals and warm handoffs to partnering agenciesEnsure continuity of care and follow up on referralsTrack client progress and document servicesEducate clients about available benefits and rightsComplete all documentation in accordance with OMH, DOH, HUD and HDSW standards.Track outcomes and service utilization for reportingEducation & Experience Requirements:Bachelors degree in Social Work, Human Services, Psychology, Sociology, Public Health, or a related field with 2+ years of experience in case management, peer support, care coordination, or social service navigation OR associate degree with 4+ years of experience in case management, peer support, care coordination, or social service navigationRequired Skills and Qualifications:Valid drivers license requiredBi-lingual English and Spanish a plusNYS Justice Center Criminal History Background Check clearance requiredNYS Office of Children and Family Services for instances of child abuse and/or neglect clearance required.Must be legally eligible to work in the United States.Skills & CompetenciesStrong communication, engagement and motivational interviewing skillsAbility to work effectively with diverse populationsExcellent organization, time management, and follow-throughKnowledge of social service systems and community resourcesFamiliarity with UNITEUS or similar EHR systemsSalary Range: $23.07- $24.03 per hourSign on Bonus: $3000 paid out over first year of satisfactory employmentBenefits:Medical, dental, and vision insurance401k retirement benefits with employer matching at one year of employment up to 2%Paid time off including holidays, Vacation, Personal and SickLife & disability insuranceAflacUS Alliance Credit UnionTuition ReimbursementHDSW is an Equal Opportunity EmployerHow to Apply: Submit your resume and cover letter via email to jcruz@hdsw.orgInclude Social Care Navigator HDSW in the subject line.Compensation details: 23.07-24.03 Hourly WagePI65c32308967d-31181-39934469RequiredPreferredJob IndustriesOther
