Telehealth Nurse Practitioner
Telehealth Geriatric & Post-Acute Care Part-Time 24 hours/week Are you a Nurse Practitioner looking for a fully remote opportunity where you can make a meaningful impact on the lives of older adults? We are seeking experienced NPs to join a growing national after-hours care team providing telephonic and virtual support to residents in skilled nursing facilities and senior living communities. Position Highlights Fully remote/work-from-home position Part-time (24 hours/week) opportunities available Consistent, set schedule Competitive pay: $66.67/hour Meaningful patient interactions with a focus on improving outcomes and reducing unnecessary hospitalizations Collaborative support from nurses, facility staff, and physician teams Schedule Coverage is provided during evenings, overnights, weekends, and holidays. Part-Time Schedule 24 hours per week Typically 3 shifts per week Coverage Expectations Weeknight coverage between 5:00 PM and 8:00 AM Every other weekend coverage Weekend shifts are generally structured as 12-hour shifts Holiday participation required Two holiday shifts annually Responsibilities Provide after-hours clinical support via telephone and telehealth platforms Assess and manage acute changes in condition, falls, medication concerns, and chronic disease exacerbations Evaluate patients remotely and determine appropriate treatment plans and next steps Collaborate with nursing staff, facility personnel, physicians, hospitals, and other care providers Prescribe medications, including controlled substances when clinically appropriate Support treatment-in-place initiatives to help avoid unnecessary emergency department visits and hospital admissions Document all patient encounters accurately and thoroughly within the electronic medical record Follow established clinical protocols and evidence-based practice guidelines Required Qualifications Master's Degree from an accredited Nurse Practitioner program National certification as FNP, ANP, or GNP Active, unrestricted Nurse Practitioner licenses meeting one of the following regional requirements: Northeast Pennsylvania and New York Southeast North Carolina and South Carolina Midwest Any combination of two or more of the following: Iowa Missouri Kansas Ohio DEA registration or ability to obtain Minimum 3 years of Nurse Practitioner experience Experience managing acute and chronic medical conditions Strong clinical judgment and ability to work independently Proficiency with electronic medical records and clinical documentation Preferred Experience Skilled nursing facility (SNF) experience Long-term care or senior living experience Geriatric medicine Urgent care Emergency medicine Post-acute care Chronic disease management Advanced illness and end-of-life care discussions Telehealth experience Ideal Candidate We are looking for a clinician who is: Comfortable managing multiple patient interactions simultaneously Adaptable and flexible in a fast-paced environment An excellent communicator with strong problem-solving skills Confident making independent clinical decisions Passionate about improving care for older adults If you're seeking a flexible remote opportunity where your clinical expertise can directly improve patient outcomes during critical after-hours periods, we encourage you to apply.
