This personis the primary case manager for the youth assigned to them and is responsible for ensuring that all services specifiedin the youth's case plan are delivered as required. This individual is expected to take an active role in the developmentand fostering of relationships between youth and staff. This person is expected to display and maintain a highstandard of ethical behavior and professionalism and function as a member of a team in providing a balancedapproach to planning and assessment for VisionQuest youth.Essential FunctionsCase Management Operations:Oversee the comprehensive management of case services, ensuring strict adherence toprotocols for youth evaluations and assessmentsMaintain meticulous documentation to guarantee compliance with VisionQuest, State, andFederal regulationsConduct thorough reviews of all assessments and related documents for precision andcorrectness prior to submissionEnsure the timely and accurate submission of all reports and deliverables in accordancewith state licensing requirements and VisionQuest's policies and proceduresCommunication and Coordination:Maintain regular contact with various stakeholders involved in the youth's careCoordinate with service providers and community resources to ensure comprehensive careAssist with referrals and coordination of any specialty servicesMaintain professional communication with all stakeholders, including family membersand guardiansService Delivery:Monitor and review youth Program Plans and progressUnderstand the individual strengths and needs of each youth on caseloadProvide supervision and support for youth activities as requiredMaintain security and confidentiality of all youth information including recordsCompliance:Document all case management actions and communications as requiredMaintain an in-depth understanding of VisionQuest and licensing policies and proceduresMonitor timeliness and maintain content of all reports, letters, and notificationsOther tasks and responsibilities as assigned by supervisorsOther tasks and responsibilities as assigned by supervisors.Skills & AbilitiesExcellent interpersonal communication skills with ability to listen activelyCommitment to professional development and proactive researchAbility to work independently and as part of a teamStrong computer skills (Microsoft Office, Google Workspace, Google Drive) with the ability to learnnew softwareExcellent writing skills including the ability to produce comprehensive narrative reportsEducation & ExperienceBachelor’s degree in the behavioral sciences, human services or social services fields.Child welfare and/or case management experience is strongly encouraged.A minimum of one year of related work experience, preferred.QualificationsMust be 21 years of age or older.Flexible schedule (evening, weekends, holidays) to facilitate program services.Cleared tuberculosis test results.May be required to pass a drug test.Cleared background checks from appropriate entities, including Child Abuse and Neglect and FBIcriminal background.Obtain and maintain CPR/First Aid and Crisis Prevention Intervention (CPI) certification.Must have a valid driver’s license and be eligible to drive to facilitate program services as required.Working Conditions/Physical RequirementsMay lift and carry up to 10 pounds frequently, up to 20 pounds occasionally, up to 50 poundsinfrequently.Performs tasks that may involve standing or walking 50% of the day; sitting 50%of the day.May bend, stoop, reach and grasp as necessary.Must be able to provide First Aid and CPR.Must be able to perform physical holds as instructed in Crisis Prevention Intervention training(CPI).The Pay Range For This Role Is22 - 22 USD per hour(410 Morningstar Youth Academy )
