And above all, it means exceptional care, simply delivered - and we couldn't do it without you.Make a DifferenceThe Registered Nurse Navigator for Cardiothoracic Surgery acts as a single point of contact and a personal guide or liaison for patients undergoing cardiac surgery. They coordinate care, educate patients and families, facilitate communication among the healthcare team, and provide ongoing support throughout the patient's journey, from pre-op preparations to post-op recovery. This role helps improve patient outcomes and experience by ensuring seamless care transitions and increasing patient understanding and adherence to treatment plans. The office is located on the north side in Indianapolis, Indiana.Exceptional Skills and QualificationsGraduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience. Bachelor of Science in Nursing (BSN) preferred.Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).Two years or more of nursing experience required.Cardiology or surgery experience preferred.Experience in care coordination or patient navigation preferred.Excellent communication skills.Excellent team skills.Compassionate and caring.Accountable and self-directed.EPIC experience a plus.Why Community?At Community Health Network, we build teams that deliver exceptional care through empathy, communication and collaboration.
