Social Worker, Care ManagementMonday-Friday, 8am-4:30pmFull TimeMeritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge.
