They address contextual factors impacting a person’s overall health and well-being. Assessing and providing interventions that pertain to health-related social needs offer an augmented transitional plan by improving access to psychosocial supports and community resources. The social worker increases relationship-based care that allows for improved communication and shared decision-making among the care team. The social worker will collaborate closely with inpatient case management and additional care team members to enhance patient transitions and experiences. Their role includes psychoeducation, crisis intervention, supportive counseling, resource facilitation and strong advocacy in supporting a person's rights to self-determination to meet their goals. This role parallels and enhances the discharge plans of the inpatient case management team.$27 to $42/ hourExact compensation may vary based on several factors, including skills, experience, and education.Benefit packages for this role will start on the 31st day of employment and include medical, dental, and vision insurance, as well as HSA, FSA, and DCFSA account options, and 401k retirement account access with employer matching.
