Housing Care Coordinator
Job TypeFull-TimePosition Summary:The Housing Care Coordinator is responsible for ensuring that clients are provided with safe and healthy housing conditions. This position plays a crucial role in conducting client intakes, facilitating in-person education sessions, performing comprehensive assessments for wrap-around services, coordinating care, and conducting pre- and post-intervention surveys along with client satisfaction surveys to measure outcomes and program effectiveness.Job Duties:Conduct client intakes, including eligibility screening, initial documentation collection, and program orientation.Evaluate the economic and psychosocial needs of clients during in-home visits.Engage with clients through outreach, supporting them as they participate in the Healthy Housing program.Collaborate with other service providers to ensure coordinated, goal-oriented care, emphasizing care transitions.Forge community ties, especially in high-demand areas of Omaha, to identify potential clients and champion their needs.Educate clients on healthy housing principles and environmental health hazards like asthma and radon.Coach clients on self-advocacy, smoking cessation, leveraging benefit opportunities, and mastering daily living activities.Ensure timely and accurate maintenance of documents, records, and reports in line with policies, using a cloud-based platform as well as Microsoft 365.Act as a point of reference for team members regarding community partners and services.Partner with Environmental Service Coordinator for the placement and collection of air quality monitors.Conduct pre- and post-intervention surveys, environmental health assessments, and client satisfaction surveys to measure program outcomes, client progress, and service effectiveness.Perform other duties as needed.Required Skills/Abilities:Knowledge of the principles of case management and trauma informed care.Excellent verbal and written communication skills.Excellent interpersonal and conflict resolution skills.Excellent organizational skills and attention to detail.Excellent time management skills with a proven ability to meet deadlines.Strong critical thinking skills.Ability to act with integrity, professionalism, and confidentiality.Ability to adapt in a rapidly changing environment.Regular and predictable attendance, and promptness for work.Commitment to uphold the mission, vision, and values of Lutheran Family Services.Support the organization’s objective to be an inclusive and accessible workplace.Position Competencies: Attention to DetailEmpathyCritical ThinkingOrganizational SkillsBuilding RelationshipsEducation and Experience:Fluency in spoken and written English required; proficiency in Spanish speaking skills strongly preferred to effectively serve clients and families.High school degree or equivalent is required. Bachelor’s degree in social work or related field preferred.Two years’ knowledge, familiarity or experience in case management services required.Demonstrated skills and competencies to work with individuals regarding a variety of behavioral health needs.Knowledge of the functions and services of community organizations and related human services.Physical Requirements:Able to safely provide services in office and field-based environments, including client homes with stairs, narrow hallways, and varied household conditions. Frequent walking, standing, bending, kneeling, and reaching as required by service delivery.Valid driver’s license, liability auto insurance, and ability to drive a personal vehicle for agency business is required.Travel to various office locations and within the community to support client services, including meeting with clients, collecting required documentation, and coordinating services, as needed and as applicable by region.Must be able to work a flexible schedule, including evenings and/or weekends (as needed).Occasional lifting of office items no more than fifty (25) pounds.Lutheran Family Services is an equal opportunity employer.
