Office Location: Baden Business OfficeWork Hours: Primarily M-F business hoursThe Population Health Pharmacist is responsible for optimizing medication therapy outcomes across targeted patient populations through evidence-based clinical interventions, medication management, and collaborative care strategies. This role supports organizational population health initiatives by improving quality metrics, reducing healthcare costs, enhancing medication adherence, and promoting safe and effective medication use. Supports the mission, vision, strategic plan and service strategy of Heritage Valley Healthcare Network.RequiredDoctorate of PharmacyLicensed or eligible for licensure in the state of PennsylvaniaMust have the ability to expertly communicate verbally and in writing including the use of electronic mail.Must possess excellent human relations skillsMust have the ability to interact in a professional mannerMust have the ability to delegate and motivate others within the department to act on performance improvement opportunitiesMust be adaptable to changeMust know and understand the law as it applies to outpatient practicesPreferredManagement experience preferredExperience and knowledge of population health strategy.Ambulatory pharmacy experienceCompetency with Microsoft Office suiteUnderstanding of value based reimbursement programsAdvanced degree in Pharmacy or Business
