Hospice Clinical Social Worker MSWThe Hospice Clinical Social Worker Master of Social Work (MSW) coordinates the psychosocial care and services for assigned patients in collaboration with the patient family physician patient care team and payers. The Hospice Clinical Social Worker MSW utilizes advanced psychosocial skills to facilitate the coordination of care through assessment planning intervention and evaluation. The Hospice Clinical Social Worker MSW promotes and evaluates the effective utilization of resources using current knowledge awareness of community services and serving as a role model to achieve optimal clinical and resource outcomes.PRIMARY ACCOUNTABILITIES:Conducts psychosocial assessment to screen for patients/families at risk for crisis abuse financial or environmental issues collaborates with Registered Nurse (RN) Case Manager ancillary staff and licensed practitioners in facilitating continuity of care for the patient ensuring to completes mandatory reporting processes as required by regulations and best practices.Assesses and documents in electronic medical record (EMR) patient and family needs and works with interdisciplinary team to identify possible psychosocial needs.Provides therapeutic or crisis intervention counseling with patients and families supportive services for End of Life Issues Terminal Care and Bereavement issues counseling and researches placement options depending on patients needs as well as the delivery of therapeutic interventions with patients and families as needed.Documents professionally accurately timely and according to Hospice of Health First policies and procedures identifying and notating necessary transitional care needs (LTC facility respite and transfer to inpatient unit).Communicates with the patient caregiver and interdisciplinary team regarding issues related to payer or financial issues ensuring to maintain comprehension and awareness of payer/reimbursement practices and regulations affecting the patient's plan of care.Shares pertinent patient information and plan of care effectively to appropriate team members collaboratively determines desired end of life goals and communicates options to patient/family physicians and staff members.Identifies "at risk" patient populations strengths of patient/family support systems and structure or process breakdowns and improvement opportunities distinguishes risk management and quality issues and intervenes or refers as indicated.Intervenes and recommends appropriate referrals to resolve barriers to patient progression collaborating with other disciplines as directed ensuring to initiate appropriate referrals to Ethics Committee Risk Management or legal services as appropriate.Assures compliance with regulatory standards of care at both the state and federal levels while verifying compliance with the National Patient Safety Goals (i.e. use of two patient identifiers when assessing patient information and in the performance of job duties).Recommends supportive and therapeutic interventions as indicated to ensure a cost effective safe and efficient plan of care and quality patient outcomes.Reinforces and documents education with patient/family on psychosocial aspects of care and expected outcomes and provides and notates education for members of the team regarding appropriate services and referrals available for patients and families.Participates in resolution of legal issues such as guardianship power of attorney etc. and assists with the implementation of advanced directives.WORK EXPERIENCE:MINIMUM QUALIFICATIONS:Education: Masters Degree in Social Work (MSW).Work Experience: None.Licensure: Drivers License and clean Motor Vehicle Report (MVR) maintained.Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card prior to start date and maintained.Background Check: Satisfies the State of Florida Agency for Healthcare Administration (ACHA) Level II background screening.Skills/Knowledge/Abilities: Proficient in Microsoft Office Teams Outlooks Word etc..Demonstrated ability to work in various healthcare systems (e.g. Epic EHR/EMR)Ability to deal with multiple demands in a short timeframe meet deadlines and be a self-starter.Committed to quality patient care and maintaining the professional standards of Social Work.Ability to work in a team environment effectively.Ability to relate to and work with the ill elderly and emotional individuals within the hospice and community settings with compassion and professionalism.Demonstrate knowledge of hospice philosophy and services progression of terminal illness cultural diversity grief process and funeral planning.Participates in patient care conferences as necessary and in patient team conferences in assigned areas.Demonstrates professional accountability through supporting patient's rights informed consent and advanced directives.PREFERRED QUALIFICATIONS:Work Experience: Three (3) years experience of social work.Prior experience in acute care Hospice or Home Care environments.Licensure: Licensed Clinical Social Worker (LCSW) licensure in the State of Florida.PHYSICAL REQUIREMENTS:Physically agile may require walking standing running bending stooping kneeling climbing or crouching frequently.May occasionally include lifting or moving objects up to 50 pounds with or without assistance.May require moving in different positions to accomplish tasks in various environments including tight and confined spaces including reaching out and above shoulders or overhead.May lift turn pull push or ambulate people and/or equipment with or without assistance up to 90 of the assigned shift.May be exposed to inside and outside environments with varied temperatures air quality lighting and/or low to loud noise.May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines.Communicating with others to exchange information.Visual acuity and hand-eye coordination to perform tasks.Workspace may vary from open to confined.May require travel to various facilities within and beyond county perimeter may require
