Monarch Hospice takes pride in serving several counties in the western Pennsylvania communities with empathy and expertise.Role DescriptionThis is a full-time, on-site role for a Hospice Liaison at Monarch Hospice, which is located in Lower Burrell, PA. The Hospice Liaison will build relationships with healthcare providers, coordinate patient referrals, and serve as the primary contact between Monarch Hospice and community partners. Responsibilities include educating facilities and familes on hospice and palliative care services, assisting families in accessing care, and ensuring patients receive appropriate end-of-life support. The role requires collaboration with the clinical team to maintain a seamless patient experience. QualificationsIn-depth understanding of Hospice Care, End-of-Life Care, and Palliative CareExperience in Sales, particularly in healthcare or related fieldsFamiliarity with Home Care practices and servicesStrong interpersonal and communication skills to engage with healthcare providers, patients, and familiesProven ability to build and maintain professional relationshipsKnowledge of federal, state, and local regulations related to hospice and home care is a plusHealthcare management, nursing, social work, or a related field is preferredValid driver’s license and reliable transportation for community visits
