Responsibilities And Duties Implement plan of care in consultation with physical therapist and other care-team members. Communicate findings or changes in regard to patients back to the physical therapist and other care-team members. Educate patients and their families in basic home safety and medical equipment safety. Solution driven, self-motivated, and inspiring to patients to reach their goals. Work independently without direct oversight. Able to discern when to call for support and communicate challenges. Familiar and comfortable with technology. Qualifications & Skills Associate Degree in Physical Therapy Current Certification in BSL/CPR Home Health Experience Valid Texas Driver's License and Automobile Insurance Knowledge of WellSky EMR software is a plusSalary: TBD
