PT PM Shift Caregiver/Med TechRole: Caregiver/Med TechResponsibilities:Personal cares to include assistance in ADL's, mobility, positioning, bathing, toileting and dinning assistance. Also provides assistance with preparing meals, laundry and maintaining a clean, safe physical environmentObserving and reporting of any changes of condition to managementAnswer call lights timely and properlyAdheres to the written resident care planPerform delegated tasks as instructed by the RN, Residence Director and RCC.Assists with housekeeping duties throughout the facility (vacuuming, dusting, sweeping, cleaning, etc) as directedAssist with activities for residents as directedDocuments observations and activity participation in an accurate, complete and timely manner.Maintains professional appearance and attitudeMaintains excellent work attendance; is punctual for work shift.Be a team playerPerforms any other miscellaneous tasks as assignedAttends all mandatory staff meetings and in service trainingsRequirements:High school diploma or GED with ability to read and comprehend written instructionsSatisfactory criminal background and reference checkSkills in rendering care and service to seniors with or without dementiaDisplay mature behavior in speech and actionNegative communicable disease screen (including TB)Ability to maintain acceptable work attendance record /follow work scheduleAbility to organize and utilize time effectivelyNegative drug screen (if performed)Work schedule:8 hour shift10 hour shift12 hour shiftWeekend availabilityABridge Care Concepts, LLC - ABridge Care Cottage of Chilton 323 Field Ln, Chilton, WI 53014, USA
