Care Coordinator

Compensation Range: $22/hr - $25/hr commensurate with experience Benefits: We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. This money is available for tuition loan reimbursement, tuition costs, and text books. Overview:The Care Coordinator addresses HIV healthcare disparities by facilitating access to care and other services through a client-centered, holistic, and comprehensive approaches to meeting the needs of people living with HIV (PLWH) with team-based care management. The program makes use of a navigation approach to identify, advocate for, and coordinate the complex healthcare and social service systems necessary to ensure improved outcomes among people living with HIV (PLWH). Responsibilities:Monitoring navigation services for each patient on their caseload to ensure that they are conducted according to program standards.Leading comprehensive assessments, organizing case conferences, and developing comprehensive service plans.Conducting case-finding activities by working from reports generated from clinic records and reaching out to patients who may have missed an appointment, been out of care, or have high viral loads.Coordinator completes the following:Provides client - centered, culturally and linguistically competent servicesSupports the delivery of preventive and primary care to enrolled clients by facilitating adherence to scheduled appointmentsProvides coordination among Health Centers and Supportive Services programs as well as other units in the organizationConducts assessment and re-assessments of client needs as neededDevelops individualized service plans for clientsCoordinates and participates in case conferences as a member of the integrated teamTravels to various Housing Works Community Healthcare site i.e. Cylar House Community Healthcare, East New York Community Healthcare, and Downtown Brooklyn Community Health care.Participates in program and agency quality initiative projects Requirements:Bachelor’s degree and at least 2 years of case management experienceKnowledge of and experience with program development and clinical practiceComputer literacyAbility to interact with inter/disciplinary team including medicine, social work, psychiatry, nursing, administration, and other paraprofessionalsAbility to work with and understand issues of people with HIV/AIDS, mental illness and substance use populationsKnowledge of and experience with mediation and conflict resolutionGood verbal and written communication skillsEmbraces a harm reduction approach to working with underserved populationsExperience with eSHARE and/or eClinical Works is preferred Job candidates should be aware that scammers may pose as employers and create fake job postings in order to extract personal information from individuals for financial gain.
