Develops and implements individualized care plans based on these assessments.Nurse Triage: Performs initial triage to prioritize patient care by evaluating the urgency of their conditions. Determines appropriate next steps for treatment or referral, ensuring timely and effective care.Care Coordination and Communication: Collaborates with physicians, specialists, and other healthcare providers to coordinate care, ensure effective communication, and manage patient treatments and follow-ups.Patient Education and Support: Provides education to patients and their families about managing health conditions, medication administration, and lifestyle changes.
