Case Manager/ppProvides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans/ppPerforms treatment(s) ordered by the physician and delegated by the R.N. Case Manager./ppObserves signs and symptoms and reports them to the R.N. Case Manager./ppNotes changes in physical or emotional status of clients./ppPromotes client and caregiver independence in self-care/ppUtilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of data/ppFollows Agency policies regarding universal precautions, and safety guidelines/ppFulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services./ph3Qualifications/h3pEducation and Experience:/ppHolds a current Licensed Practical Nurse license in Virginia/ppHas a minimum of one year of nursing experience/ppHas current CPR certification/ppHas excellent communication, organizational, and human relations skills/ppIs able to function with minimal supervision/ppIs able to manage multiple projects/changing priorities/ppIs able to employ critical thinking skills/p/div
