Chester also offers local history and culture through places like the Liberty County Museum and Liberty Village Arts Center and Gallery, adding to the town's welcoming and authentic small-town feel.Position ResponsibilitiesDevelop, implement, and revise individualized, goal-directed physical therapy plans of care through skilled evaluation and ongoing assessmentPerform comprehensive patient assessments reflecting functional, physical, mental, emotional, psychosocial, and environmental statusDevelop and maintain updated individualized care plans that include goals, interventions, medications, and visit frequency ordersPlan, communicate, supervise, and delegate therapy services in a competent and efficient mannerProvide treatment according to established physical therapy standards and department policies and proceduresActively participate in the rehabilitation process, including admission and discharge evaluations, daily assessments, and individualized goal-settingDemonstrate excellent customer service and communication with internal and external customersComplete all required documentation in a timely manner according to payer guidelines and Montana physical therapy practice guidelinesSupport rehabilitation services through participation in departmental, facility, and community programs or projectsAchieve productivity standards according to department protocolMaintain regular and consistent attendance as scheduled by department leadershipQualificationsMinimum of one (1) of the following required:Current Montana Physical Therapy licenseORGraduate of an accredited Physical Therapy program and must successfully obtain Montana Physical Therapy license within thirty (30) days of hire Additional requirements: Current HS AED CPRExcellent organizational skills and attention to detailSelf-starter with strong critical thinking skills and the ability to prioritize effectivelyAbility to work both independently and as part of a teamCommitment to maintaining confidentialityExcellent verbal and written communication skills, including the ability to communicate effectively with a variety of audiencesStrong interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacyWorking knowledge of Microsoft Office Suite and the ability to learn additional software as neededWhy Join Logan Health?This PRN opportunity offers the chance to make a meaningful difference while serving a rural community that truly values healthcare professionals.
