Community Support Worker
Job Summary
We are seeking a compassionate and motivated Community Support Worker (CSW) to support individuals and families receiving behavioral health and community-based services. The CSW will assist clients in building daily living skills, accessing community resources, improving social functioning, and working toward individualized goals outlined in service plans. This role requires strong interpersonal skills, professionalism, and the ability to work independently in the community.
Key Responsibilities
Provide direct community-based support services in accordance with individualized service plans
Assist clients with skill development related to daily living, socialization, organization, and community integration
Support clients in accessing community resources such as medical appointments, social services, education, employment, and recreational activities
Provide coaching, encouragement, and modeling of healthy coping and life skills
Maintain accurate, timely, and compliant documentation in accordance with agency, Medicaid, and state requirements
Coordinate and communicate with clinical staff, care coordinators, therapists, and other team members
Transport clients as needed using a reliable personal vehicle
Maintain professional boundaries and uphold client confidentiality at all times
Participate in team meetings, supervision, and required trainings
Pay: $37,000.00 - $50,000.00 per year
