Seal Beach Health and Rehabilitation Center - Pay: $24-27/hr DOE1 year experience preferredPOSITION SUMMARYThe purpose of your job position is to assist in the implementation and evaluation of the social services programs as they meet the needs of resident care plans in accordance with current federal, state and local standards that govern the facility, and as assigned by the Director of Social Services.The Social Services Assistant will treat each resident fairly, kindly, and show them dignity and respect. They will knock on the resident's door before entering. They will greet them upon entering their room. They will refer to each resident by their name. They will communicate clearly, empathetically, and effectively when speaking to residents, family members, visitors, all staff and governmental agencies. They will work cooperatively with all departments and multidisciplinary teams. They will demonstrate patience, initiative and willingness to assist residents that may be difficult. They will relate all pertinent information concerning a resident's condition to a charge nurse when required. They will be committed to always doing the right thing.ESSENTIAL DUTIES AND RESPONSIBILITIESAssisting Director of Social ServicesMaking phone calls to set up transportation or doctor appointments for residentsProviding residents and family Medicare/Medicaid and financial information when requestedMaintaining accurate social service progress notesUpdating the social services directoryObtaining information from community social health and welfare agencies in order to meet the resident's needsPlanning and organizing time and duties to efficiently maintain caseloadReporting any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediatelyProtecting each residents file and data with confidentially per HIPAA standardsAbiding with all facility policies and procedures including not disclosing user ID codes and passwordsEvery effort has been made to identify the essential functions of this position.
