Home Care Occupational TherapistThe Home Care Occupational Therapist provides in-home occupational therapy services to support patients in improving independence, safety, and function in daily living activities. This role involves evaluating patients in their home environment, developing individualized treatment plans, and delivering skilled interventions to promote optimal recovery and quality of life.Duties and ResponsibilitiesPerform occupational therapy evaluations in patients' homesEstablish individualized goals and treatment plans based on patient needs and functional limitationsProvide skilled therapy to improve independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs)Assess home environments and recommend safety modifications to reduce fall risk and improve accessibilityTrain patients and caregivers in adaptive techniques, safety strategies, and use of assistive devicesRecommend and educate on appropriate adaptive equipment and durable medical equipmentDocument evaluations, treatment interventions, and progress notes in a timely and accurate mannerCollaborate with interdisciplinary team members including physicians, nurses, and therapistsMonitor patient progress and adjust treatment plans as neededEducate patients and families on functional maintenance, safety, and injury preventionEnsure compliance with all regulatory guidelines, agency policies, and documentation standardsQualificationsDegree in Occupational Therapy (BS, MS, or Doctorate) from an accredited programCurrent Occupational Therapist license in the practicing stateNBCOT certification (if required by state regulations)Valid driver's license and reliable transportationAbility to travel within assigned service area to patient homesPreferred QualificationsExperience in home health, rehabilitation, or outpatient therapyExperience working with older adults or medically complex patientsFamiliarity with electronic medical record (EMR) systemsStrong independent clinical judgment and problem-solving skillsExcellent time management and organizational skillsWork EnvironmentField-based role requiring daily travel to patient homesFlexible schedule based on patient needs and caseloadVaried home environments requiring adaptability and clinical judgmentPhysical activity may include standing, bending, and assisting patients with mobility tasks
