Our mission is to deliver personalized, community-based care that empowers members to lead healthy, independent lives.Role DescriptionThe Clinical Pharmacy Specialist will play a critical role in ensuring medication safety and effectiveness for our members. This is a full-time, on-site position with some remote work opportunities located in Milwaukee, WI. Responsibilities include conducting medication reviews, developing and implementing a Part D formulary, collaborating with healthcare providers to optimize patient outcomes, and managing the MTM Program while completing CMRs and Targeted Medication Reviews. The Specialist will also ensure compliance CMS regulations, reviewing and providing data validations, and heading the P&T committee.Pharmacist Job Specific Responsibilities  Medication safety Patient safety monitoring, Overutilization monitoring system, drug management program reportingCollaboration & Communication Facilitates collaboration and communication between departments through various means, such as conducting and attending meetings, site visits, having one-to-one communications with operations management staff, and generally being visible and accessible to colleagues and staff. Special Assignments & Projects Coverage Determinations Participation in P&T committeeFormulary development and submission to CMSMedication Therapy ManagementPharmacy student preceptorFWA monitoring and reportingCoordinates delegates or performs special assignments and projects as requested by leadership staff. Pharmacist Qualifications and Requirements ExperienceRequired A minimum of three(3) years clinical pharmacy practice required. Good standing pharmacy License in Wisconsin. Preferred A minimum of one (1) year of pharmacy managed care and/or prescription plan administration preferred  Education - PharmD or Bachelors of Science in Pharmacy Required. Must have active pharmacy license in the state of WI.
