Job Description & Requirements Specialty: Home Health Discipline: RN Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Title: Registered Nurse Unit: Home Health Location: Meridian, ID, 83642 Shift: Mon-Fri 8:30 Am -5 Pm weekend call rotation Shift Length: 8 hours Guaranteed Hours: 40 hours per week Duration: 13 weeks Position Summary: We are seeking an experienced Registered Nurse (RN) – Home Health with a minimum of 2 years of experience to provide high-quality patient care. The RN will be responsible for conducting comprehensive assessments, determining Medicare homebound eligibility, and identifying the appropriate skilled service for each visit (hands-on, direct care, observation/assessment, education, or medication & evaluation). The role requires strong proficiency in OASIS documentation and care planning. Candidates must have prior home health experience and be available for rotating weekend and evening on-call coverage. Roles & Responsibilities: Perform comprehensive patient assessments and determine eligibility under Medicare homebound criteria Develop, implement, and update individualized care plans based on patient needs Identify and deliver appropriate skilled services (hands-on care, direct care, observation/assessment, patient education, medication & evaluation) Complete accurate and timely OASIS documentation in compliance with regulatory standards Administer medications, treatments, and monitor patient progress and response to care Coordinate with physicians and interdisciplinary teams, and participate in rotating weekend/evening on-call coverage Skills: Strong understanding of Medicare homebound criteria and eligibility determination Proficiency in OASIS documentation and regulatory compliance Skilled in patient assessment, care planning, and clinical decision-making Ability to determine and deliver appropriate skilled services (hands-on, observation, education, medication & evaluation) Effective communication and patient/family education skills Ability to work independently with strong time management and adaptability in home settings Documents Required: Valid ID State RN license ACLS Certified through AHA BLS Certified through AHA COVID card Please submit your resume to or Call/text .
