Job Title: Outpatient Infusion PharmacistLocation: Hood River, ORType: ContractShift & Schedule: Day Shift | 4x10-Hour Shifts | 7:00 AM - 5:00 PMBlock Scheduling Available Position Overview: Navitas Healthcare, LLC is seeking Outpatient Infusion Pharmacist for an exciting Travel or Local job in Hood River, OR. Responsibilities Review, verify, and dispense medications in accordance with physician orders and established protocols Prepare and oversee infusion-related medications for outpatient administration Ensure safe and accurate medication preparation, dosing, and dispensing practices Monitor medication therapy for effectiveness, safety, interactions, and contraindications Maintain compliance with state, federal, and organizational pharmacy regulations Collaborate with physicians, nurses, and healthcare staff to optimize patient treatment plans Provide medication counseling and education to patients as appropriate Support infusion therapy management and medication-related clinical decision-making Monitor patient medication profiles and identify opportunities for therapy optimization Maintain accurate medication records and documentation within the EPIC electronic medical record system Adhere to infection control, medication safety, and quality assurance standards Participate in pharmacy audits, medication reconciliation, and process improvement initiatives Qualifications Doctor of Pharmacy (PharmD) or equivalent Pharmacy degree required Active Pharmacist License required BLS preferred based on facility requirements Minimum 1 year of outpatient infusion pharmacy experience required Experience in infusion medication preparation and management Strong understanding of pharmacology, medication safety, and infusion therapies EPIC EMR experience preferred Excellent communication and interdisciplinary collaboration skills Strong attention to detail and clinical judgment For more details contact Call / Text at .
