We are a 5-Star Medicare-rated concierge home health agency, ranked among the top 5% of all agencies nationwide.________________________________________Position SummaryThe Patient Care Coordinator (PCC/Intake) serves as the central hub of communication, coordination, and scheduling for our home health operations. This role ensures that field clinicians can deliver timely, high-quality care by supporting daily scheduling, intake processing, order management, supply logistics, and insurance verification.The PCC interacts regularly with physicians’ offices, case managers, discharge planners, and internal clinical leadership.
