Care Coordinator - Integrated Health Home

JOIN OUR TEAM!As a part of our integrated Health Home team, the Care Coordinator is responsible for following clients with identified mental health needs in the primary care setting. Maintain regular contact with clients to assist in locating, coordinating and monitoring psychosocial, habilitative and medical services designed to meet the client's individual needs.What You'll DoServes as a member of the IHH interdisciplinary team Provides IHH program care management services to eligible patients, in which an individual's needs are identified and the psychosocial, habilitative and medical services designed to meet those needs are locatedCoordinated and monitored in collaboration with other professional staffFollow clients with identified mental health needs in the primary care setting and monitor their response to treatment.Maintain regular contact with clients in order to assess the client's treatment adherence and administer the mental health assessment tool to track symptom severity.What You'll NeedBachelor's degree in a related fieldMinimum one year casework/case management experience working with the long-term mentally ill populationExcellent written & verbal communication skills, computer skillsValid driver's license with a driving record that will allow insurabilityBilingual skill is a plusStrong organizational and time management abilities importantWho You AreHumble.
