This role focuses on delivering high-quality clinical services, including admissions and ongoing patient care management, while ensuring accurate documentation and collaboration with the care team. The ideal candidate is adaptable and comfortable working a flexible schedule that may include days, evenings, weekends, and on-call rotations.Schedule & Work Details:Schedule: Flexible shifts including days, evenings, and weekendsWeekly Guarantee: 36 hoursProductivity Expectation: 30 pointsOn-Call/Weekend Requirement: Every 3 to 4 weeks when fully staffedMileage Reimbursement: Not providedPay Rate: $51 to $53 per hourKey Responsibilities:Perform Start of Care (SOC), Resumption of Care (ROC), and recertification visitsConduct comprehensive patient assessments and develop individualized plans of careMonitor patient conditions and update care plans as neededProvide skilled nursing interventions in accordance with physician ordersMaintain accurate, timely, and compliant documentationCollaborate with physicians and interdisciplinary team members to ensure continuity of careEducate patients and caregivers on treatment plans, medications, and disease managementManage a dynamic schedule and caseload efficiently while meeting productivity expectationsRequired Qualifications:Active Registered Nurse (RN) licenseStrong clinical assessment and documentation skillsAbility to work independently and manage a flexible scheduleExperience with SOC, ROC, and recertificationsPreferred Qualifications:Prior home health experienceStrong time management and organizational skillsExcellent communication and interpersonal skillsAbility to adapt to changing patient and scheduling needsPride-Health offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.Equal Employment OpportunityPride-Health is an equal opportunity employer.
