Chaim Gitelis DO PC is a growing primary care practice committed to integrating behavioral health into everyday patient care. We operate under the evidence-based Collaborative Care Model (CoCM), embedding mental health support directly within our primary care framework to improve access, outcomes, and the overall patient experience. We are expanding our behavioral health team and seeking a passionate Psychiatric Nurse Practitioner to serve as our consulting psychiatric provider for the CoCM program.
Position Summary
As the Psychiatric Consultant NP for our CoCM program, you will provide clinical oversight and consultation to our Behavioral Health Care Manager and attending primary care providers. You will review complex behavioral health cases, make treatment recommendations, and ensure patients receive high-quality, evidence-based psychiatric care — all via telehealth. This is a meaningful opportunity to shape and lead an integrated behavioral health program with a dedicated, collaborative team.
Key Responsibilities
* Participate in weekly or biweekly case review meetings with the Behavioral Health Care Manager and primary care providers to discuss patient progress and adjust treatment plans
* Review patient registries and provide cased-based psychiatric consultation on patients not improving as expected ("treat-to-target" model)
* Provide recommendations on psychiatric medication management, including initiation, adjustment, and titration
* Collaborate with the care team to develop and update individualized care plans
* Support the BHCM in delivering evidence-based interventions and monitoring patient outcomes (PHQ-9, GAD-7, and other validated tools)
* Ensure documentation meets HIPAA, CMS, and CoCM billing compliance standards
* Maintain active communication with the interdisciplinary team via telehealth platforms and EHR
* Contribute to quality improvement efforts and outcome tracking for the CoCM program
Required Qualifications
* Active, unrestricted Nurse Practitioner license (Psychiatric-Mental Health NP preferred; Adult or Family NP with substantial psych experience considered)
* Board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) preferred
* Active DEA license with prescriptive authority
* Minimum 2 years of clinical experience in psychiatric or behavioral health settings
* Experience in or strong familiarity with the Collaborative Care Model (CoCM) or integrated behavioral health
* Active credentialing/enrollment status with Medicare and Medicaid (or willingness to enroll)
* Proficiency with EHR systems and telehealth platforms
Preferred Qualifications
* Prior experience as a psychiatric consultant in a CoCM or primary care behavioral health setting
* Experience working with nursing facility or geriatric populations
* Familiarity with CMS billing requirements for CoCM (CPT codes 99492, 99493, 99494)
