About the JobPosition Summary:The Converse Home is a nonprofit assisted living community in Burlington, committed to providing high-quality, resident-centered care in a safe, nurturing environment where residents can thrive. Our compassionate team is at the heart of everything we do. We are seeking an experienced and compassionate Social Worker (MSW) to support our residents. This role is essential in promoting residents' emotional well-being, dignity, and quality of life by providing counseling, advocacy, care coordination, and family support. The Social Worker works collaboratively with residents, families, and the interdisciplinary care team to ensure holistic, resident-centered care.Key Responsibilities:Conduct psychosocial assessments and provide ongoing emotional support to residents.Assist residents with adjustment to assisted living and transitions in care.Collaborate with nursing, clinical, and administrative teams to support individualized care plans.Serve as a trusted resource and advocate for residents and families.Provide counseling, crisis intervention, and conflict resolution as needed.Support advance care planning, care conferences, and discharge planning.Connect residents and families with appropriate community and social service resources.Maintain accurate and timely documentation in accordance with regulations and organizational policies.Ensure compliance with state and federal regulations related to resident rights and social services.Facilitate Best FriendsTM and trauma-informed care training for staff.Assist residents and families in navigating healthcare systems, benefits, and community resources.Qualifications:Master's degree in Social Work (MSW) from an accredited program preferred.3-5 years of professional social work experience, preferably in assisted living, long-term care, healthcare, or geriatric settings.Current state licensure (or eligibility for licensure, as applicable).Strong knowledge of aging services, community resources, and psychosocial needs of older adults.Excellent communication, documentation, and interpersonal skills.Compassionate, professional, and mission-driven approach to care.What We Offer:A meaningful role making a direct impact in the lives of older adults.A collaborative, supportive, and resident-centered work environment.Competitive compensation based on experience and a robust benefits package that includes paid time off, Health, dental, and vision insurance as well as life insurance and a retirement plan.Come join a great team and love where you work!To learn more about The Converse Home and apply online, visit www.conversehome.comOr send your letter of interest and resume to kristen@conversehome.comThe Converse Home is an Equal Opportunity Employer.About the Company
