Description Title: Private Duty Nurse (PDN) Licensed Practical Nurse (LPN) - 1:1 Continuous Care As a Private Duty Nurse (PDN) Licensed Practical Nurse (LPN), you'll provide 1:1 skilled nursing care in a client's home-supporting longer, consistent shifts and building relationships with the client and family. As a Private Duty Nurse (PDN) Licensed Practical Nurse (LPN) you will: Provide 1:1 in-home skilled nursing care per the provider's plan of care and your nursing scope of practice, supporting extended shifts/continuous care as assigned. Partner with the client, family/caregivers, and care team to support goals of care and continuity across shifts. Assess, monitor, and document patient condition and report any changes promptly (vitals, blood glucose, intake/output, respiratory status, etc.). Administer medications and treatments (including insulin and IV fluids as ordered), with thorough, shift-based documentation. Provide skilled care such as wound care/dressing changes and support with personal care as outlined in the plan of care. Monitor for signs of infection, skin breakdown, respiratory distress, or other complications and respond according to protocols and provider orders. Provide skilled support for specialized needs as trained/authorized (e.g., trach care, ventilator support, enteral feeds) and assist with procedures per policy. Educate and support the client and family/caregivers on routines, equipment, and symptom monitoring to promote safety at home. Complete timely, accurate clinical notes and communicate shift handoff information to support continuity of care. Coordinate with the supervising RN, providers, and the office team to ensure excellent outcomes and reliable coverage. Communicate effectively with the care team to support coordination of services, supplies/equipment needs, and ongoing care planning. To qualify for a Private Duty Nurse (PDN) Licensed Practical Nurse (LPN) position with us: Graduate of an accredited practical (vocational) nursing program and current LPN/LVN license in the state(s) in which you practice. One year of clinical experience preferred (home care/private duty experience a plus) or completion of the Office's preceptor program. Current CPR/AED/BLS/First Aid certification. Reliable transportation to/from care sites and/or work locations. training may be available based on assignment. Able to provide proof of valid driver's license and auto liability insurance if assignments include driving own vehicle to transport patient. Working Conditions & Physical Effort (Private Duty): Work is performed primarily in a client's home in a 1:1 setting, with consistent shift-based coverage. Assignments may include home-based care and as needed, care in community settings (e.g., school) per the plan of care. Frequent exposure to communicable diseases, bloodborne pathogens, and/or other potentially infectious or hazardous materials and situations that require following extensive safety precautions, including the use of protective equipment. Medium physical activity may require occasional lifting, pushing, or pulling up to 50 lbs.
