Service CoordinatorWe are hiring a creative, detail-oriented, and flexible Service Coordinator for our Ghent location in Norfolk.In this role, you will support individuals by helping them set goals, manage medical needs, write plans, and handle personal finances. This position is ideal for someone who is organized, energetic, and passionate about helping others live full, independent lives.ScheduleMonday through Friday, 8:00 a.m. to 4:00 p.m.Full-Time BenefitsHealth insurance, including visionDental insurance (100 percent paid by Hope House)Life insurance (100 percent paid by Hope House)Optional benefits: additional life insurance, disability insurance, flexible spending accountsGenerous leave, including 12 paid holidays, 12 paid vacation days and 12 paid sick days401(k) planPosition ResponsibilitiesDaily Living and Skill BuildingSupport individuals with daily living tasks such as cooking, cleaning, laundry, hygiene, meal planning, grocery shopping, banking, transportation, and medication management.Use your personal vehicle as needed while providing services.Teach skills based on each person's Individualized Service Plan.Community IntegrationHelp individuals participate in community activities.Support individuals in exercising their rights and resolving concerns.Behavioral and Medical SupportProvide behavioral guidance and implement support plans.Administer first aid in emergencies and ensure prompt access to medical care.Assist individuals with scheduling and attending medical and dental appointments.Manage medications, prescriptions, and related documentation.Keep team members informed about health-related updates.Documentation and CommunicationMaintain accurate, confidential records, including: Medication NotebooksIndividualized Service Plan NotebooksPermanent RecordsShare relevant information with team members and serve as a liaison with families, agencies, and the community.Complete payroll, mileage, and other required forms.Team CollaborationAttend weekly team meetings, monthly team meetings, quarterly agency meetings, retreats, and required trainings.Support team operations and fill in for the Team Leader when needed.Carry the team pager when requested.Additional ResponsibilitiesAssist with errands as needed.Report maintenance needs.Test fire alarm systems when required.Follow all agency policies and support future policy development.Qualifications Ability to organize and prioritize work tasks.Proficiency in Microsoft Excel, Outlook, and Word.Ability to work effectively as part of a team.Ability to present yourself professionally.Ability to self-direct work and manage tasks independently.Ability to communicate clearly and effectively in both oral and written form.Ability to participate in and contribute to a participative management structure.Ability to meet the physical requirements of the position, including lifting up to 50 pounds several times per shift, climbing stairs multiple times per shift, and sitting, standing, and walking as needed.Knowledge of the field of disabilities.Skill in using behavioral support practices.Ability to complete required training, including Medication Certification, Medicaid Waiver Certification, First Aid and CPR, Mandt, and all orientation requirements.Skill in decision making, judgment, and action during emergency or crisis situations.Ability to maintain confidentiality of sensitive information.Skill in locating and accessing community resources.Must have a valid Virginia driver's license.Must have a satisfactory criminal history background check.
