Physical Therapist

Intrinsic ResponsibilitiesProvide assessments/evaluations and other physical therapy services to patients and/or clients for the purpose of injury prevention and rehabilitation as well as health promotion. Participate in activities and training to promote and attain regulatory compliance at the local, state, and federal level.Essential Job FunctionEvaluate patients/clients, and develop a plan of care with associated goals including quantifiable physical and functional targetsProvide assessments, treatments, instructions, and other physical therapy services according to the scope of practice limitations per State statuteRetain the responsibility for all delegated tasks to other licensees and support personnel. Ensure proper and adequate supervision of the Physical Therapist Assistant and support personnelParticipate in professional and facility enhancement programs and activities as required by the Director of Clinical ServicesEnsure proficient services through proper patient and equipment schedulingComply with the Facility’s Comprehensive Compliance Plan (CCP) which includes but is not limited to: Business & Clinical Operations, Environmental Health & Safety, HIPAA, Human Resources and Code of Conduct as well as all facility policies and procedures and educational requirements associated the CCPAssist with leadership of the physical therapy departmentComply with all local, state and federal regulations training and instruction requirements as well as any other standards that govern healthcare services provisionAssist with and/or carry out routine cleaning, maintenance support operations of the clinic’s physical plant, patient care equipment, and supplies in accordance with regulatory standards and EHS policies and proceduresCarry out EHS duties as designated policy and/or by the Director of Clinical ServicesUphold Crow Therapies values as the core of our work environment: Positive Attitude, Ethical, Adaptability, KindnessQualificationsGraduate of a Physical Therapy Program approved by the Commission for the Accreditation of Physical Therapy Education.Licensed to practice Physical Therapy in *Insert Name of State*A person who demonstrates the physical and mental capabilities (practical &/or written, post-training competency test) to perform the duties required of the position.A person who can meet the physical demand level of the position.Physical Demand CapacityConsistent with published industry PDC norms or minimally:Requires manual dexterity and general strength and enduranceLifting routinely loads of 5-35 pounds fromFloor to waistWaist to shoulderShoulder to overheadCarrying loads of 5-10# for 40-50’Pushing loads of 5-150# on casters, runners, or pulleysSustaining a grip for 1-2 minutes of 40-50#Guarding a “lift load” of 120-140#Exposure DeterminationHas the potential for Hazardous Substance Exposure due to work dutiesHas the potential for Bloodborne Pathogen Exposure due to work dutiesHipaa StatusHas been determined as eligible for access to/use of PHI & EPHI based on work duties and responsibilities in compliance with ‘minimal necessary’ standards
