Community Health Nurse - VNA
Home Care NurseThis is a per-diem position with requirement of 24 hours per month including one holiday and some weekend hours per year.Definition/Primary FunctionThis role provides, teaches, supervises and documents nursing care of the sick and injured at home in order to promote, maintain or restore health to prevent illness and to effect rehabilitation. The role collaborates with other healthcare providers, professionals and community representatives to coordinate care at home, evaluating the individual's and families' response to interventions.Position Qualifications (Minimum Qualifications Are Required Unless Stated Otherwise.)Experience:One-year acute nursing care preferred, hospital based preferred.Experience in home care setting strongly preferred.Experience with self-scheduling of caseload preferred.Education and Training:Bachelor of Science in Nursing recommended.Graduate of a N.L.N. accredited nursing program.License, Certification & Registration:RN License - Current license as a professional nurse within MAActive and valid driver's license.CPR required every two years.Other Requirements:Demonstrated knowledge of the principles and practices of nursing.Knowledge of and ability to apply community health principles and practice.Demonstrated ability to perform independently as a clinician with minimal supervision or assistance.History of flexibility in assuming varied assignments.Ability to exercise independent judgement.Demonstrated interpersonal skills.Ability to supervise others.Ability to maintain confidential information.Ability to develop and maintain records and communicate clinical data in verbal and written form.Willing to be flexible regarding work hours.Ability to drive an automobile in inclement weather.Must be willing to accept "on call" responsibilities.History of dependability in meeting demands of committed work schedule.
