Case Manager
Title: ATI Case Manager
Unit: Alternatives to Incarceration
Reports to: Program Supervisor
Status: Full Time; Regular; Non-Exempt
Salary: $24.73 - $30.22 per hour (approximately$45,000 - $55,000 annually)
Location: Long Island City, Queens
Days/Hours: Varies Monday Friday 9:00am 5:00pm, some evenings 11:30am -7:30pm, subject to change depending on program needs.
Position Summary:
Responsible for managing a case load from intake through discharge facilitate evidence-based groups as needed. Case management duties including individual sessions, developing treatment plans, documenting client progress for reporting to the courts, and making referrals to internal and external services.
Essential Duties and Responsibilities:
Perform general case management duties, including weekly individual sessions with clients, referrals to community-based services, developing client schedules, and treatment plans.
Facilitate evidence-based groups as needed.
Assist clients in identifying and prioritizing needs.
Provide referrals to ensure all participants concrete needs are addressed.
Ensure accurate and timely data entry of all client information into the agencys database.
Provide accurate and timely information in court reports.
Monitor participants attendance and compliance with mandate requirements.
Conduct and track clients toxicology
Participate in case conferences for optimum collaboration among departments/programs and external agencies.
Attend and participate in meetings, conferences, and training.
Engage in his/her own ongoing professional development.
Additional responsibilities as required.
Qualifications:
One year of case management experience preferred.
High school diploma or GED/HSE required.
Experience and commitment to working with mandated participants with criminal justice involvement.
Strong Time Management Skills
Strong Microsoft Office and database entry skills.
Strong assessment and engagement skills.
Strong written and verbal communication skills.
Bilingual English/Spanish a plus.
Strong organizational and leadership skills; and
